What Benefits You Receive?
STEP ONE
REGISTRATION
The patient or caregiver must first complete the Black Rose Registration Form.Â
The application will then be reviewed and verified by our staff and activated upon approval. If you'd prefer to manually fill out the form, you can download it here.
STEP TWO
After registration, you'll need to provide the medical documents from your healthcare practitioner before you can get full access to the shop and our products.
We have several options available for you to get us the necessary documents. See what situations suits you best here.Â
STEP THREE
SHOP!
Once your registration details and medical documents are verified you'll get full access to our store and allowed to purchase the quantity and for the duration that your prescription allows.
You will receive an email from us about how to complete your registration and how to set up your account.
